ANNOUNCEMENT

“Do you receive free phone service from TelAlaska through their Lifeline program? If
so, new regulations require you to recertify your eligibility each year. Lifeline
customers who do not return the self certification form by December 31st will be charged
the regular rate for service. Please return your Lifeline form to TelAlaska as soon as
possible or dial 611 for assistance. Forms are also available at their website,
